Third-string quarterback Shaun Hill will start for the San Francisco 49ers when they play Cincinnati on Saturday because Trent Dilfer is still suffering from the effects of a concussion he endured Sunday against Minnesota. Dilfer, who has been starting for the injured Alex Smith, spent several hours in a hospital after the hard hit in the first half. As he left the 49ers' complex Monday, Dilfer told reporters, "I can barely think, let alone talk."

Commentary: The 49ers are a complete mess, and it goes beyond the quarterbacks. They probably won't win another game, finishing with Cincinnati, Tampa Bay and Cleveland. That would make them 3-13 and put coach Mike Nolan squarely on the hot seat for next season if not immediately following this one.

Fantasy Football Impact: Hill isn't healthy either; he has a dislocated index finger on his passing hand. Despite that, he put together the most efficient game by a 49er QB this season, completing 22 of 27 passes against the Vikings. But no 49er -- except perhaps Frank Gore -- is worth a fantasy look at this point.
